The application process for admission into a long-term care facility is regulated provincially and can only be made through a Community Care Access Centre (CCAC).
CCAC will answer your questions and arrange the necessary assessments to make sure you are eligible for long-term care and give you the information you need to make this important decision.
If you are already in a long term care facility and wish to move to Chester Village, please contact the CCAC office in your area to speak with the appropriate placement coordinator to help facilitate this process.
